Private Person's Arrest
Having made the arrest, I agree to assist any member of the Irvine Police Department in this investigation. I also agree to provide a written statement, a detailed oral statement, and to appear in court, if necessary. I assume all liability and responsibility in this arrest.
Photo Lineup Admonition

You are asked to look at several photographs. The fact that the photographs are shown to you should not influence your judgment. The perpetrator may or may not be in the photo lineup. You are not obligated to identify anyone. It is just as important to free innocent persons from suspicion as to identify guilty parties. Please do not discuss the case with other possible witnesses or indicate in any way that you have identified someone. Take into consideration that photographs often do not depict what an individual looks like at the present time. Therefore, special effort should be made to identify the perpetrator by all physical characteristics. An identification, or failure to make an identification, will not end the investigation.

Witness Consent To Assist With Investigation
to assist them in an investigation. I hereby freely consent to this voluntary trip to the designated location, with the understanding that I have the right to refuse consent. I acknowledge that I am not under arrest, and that I am free to leave the designated location at any time I choose and return transportation will be provided to me if requested.
Field Identification Admonition
We are detaining a person for you to view who may or may not be the person who committed the crime now being investigated. The fact that this person is detained, and may or may not be handcuffed, should not influence your identification. You are not obligated to identify this person. It is just as important to free innocent persons from suspicion as it is to identify guilty persons. Please look at the detained person carefully. If you wish to see him or her walk or stand or move in any particular way, please tell the officer. Also, if you wish to see the person under different lighting conditions, please tell the officer. If you wish to hear the person speak certain words or phrases, tell an officer. Please do not talk to anyone other than police officers while you are viewing the detained person. Do not consult with or talk to other persons who may also be witnesses or have knowledge of the crime. You are to make up your own mind whether or not you can identify the detained person. After you have had enough time to view this person, please tell an officer whether you can or cannot make an identification. Please use your own words to tell the officer how certain you are of any identification you make. Remember, you are not obligated to make an identification merely because this person is detained, and it is just as important to free innocent persons from suspicion as it is to identify guilty persons

The above named agent(s) are authorized by me to take from my premises and/or my vehicle any letters, papers, materials, or other property which they may desire. This written permission is given by me to the above agent(s) voluntarily and without threats or promises of any kind.
